Job description

We're seeking RNs to work at an outpatient clinic. Must have BSN and be comfortable working with children.

Responsibilities

The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.

They are coordinating surgical schedules, providing financial assistance and counseling, discharge planning, patient and family education, DME/CPM (wheelchairs, crutches, etc) communication and planning, and being a liaison between the family and the providers.

Open on background to anyone with strong clinical experience and that have the right personality. They want someone who is a strong personality that allows them to work with a number of different people and personalities, someone that likes being with kids and likes to have fun in the workplace.

Hours: Monday-Friday, 8a-4:30pm. No weekends, nights, holidays, or on call.

Training: 3 week training program but they also work as a team so it is consistent and ongoing.

Location: Salt Lake City, onsite
