Job description

OVERVIEW

The Nurse Care Coordinator coordinates medical care for persons discharged from nursing homes. Work with staff on community support teams and members to ensure that health needs are assessed, coordinated and documented.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Monitor and facilitate primary health care and psychiatry services clients
  • Establish and oversee a process for communicating pertinent patient information and status reports community healthcare providers, including psychiatrists.
  • Conduct nursing activities such as assessments, medication administration, patient and significant other education.
  • Work with the Transition Coordinator to provide medical assessment/screenings for intake
  • Participating in project training and meetings as necessary.
  • Documenting patient care and other activities

EDUCATION

  • Bachelor's degree from an accredited school of Nursing. Master's preferred.

EXPERIENCE

  • Experience working with persons diagnosed with mental illness preferred.
  • Must communicate effectively both verbally and in writing in order to provide information to patients and healthcare personnel.
  • Demonstrated ability to function independently.
  • Computer skills required; knowledge of database functions and maintenance necessary.

SKILLS/CERTIFICATIONS

  • Licensed Registered Nurse required.
  • Current and valid driver's license required.
  • Daily access to a personal vehicle is required. Must obtain and maintain liability insurance coverage.

SCHEDULE

  • M-F 8:30 start time, end time flexible, must meet 40 hours a week.

**3-4 month contract, can turn into permanent role based on performance**
