Job description

Summary:

Are you passionate about enhancing patient care and improving health outcomes? Join our Clinical Care team, where you'll leverage our cutting-edge software platform to optimize patient well-being and streamline clinical services.

As a key team member, you will work under the supervision of a Pharmacist Team Lead and the Head of Operations, conducting outbound calls to patients, providers, and pharmacies. Your role will involve gathering health information, scheduling appointments, resolving medication-related concerns, and engaging patients to ensure they receive the best possible care.

Beyond patient interactions, you will also respond to inbound customer support requests and collaborate closely with the Clinical Pharmacists and Product team to refine workflows and enhance our platform. Your insights will serve as a vital connection between patients, healthcare providers, and product development.

We seek team members who communicate effectively, work collaboratively, remain professional under pressure, and embrace continuous learning to provide outstanding service and patient support.

What You'll Be Doing:

  • Engaging patients via phone to explain our clinical programs and services on behalf of their health plan.
  • Identifying and escalating potential medication-related or health concerns to a pharmacist.
  • Collecting and recording provider contact details to support clinical interventions.
  • Gathering necessary patient information to facilitate interventions.
  • Handling inbound calls from patients, providers, and pharmacies, addressing inquiries professionally.
  • Maintaining accurate records within our software platform.
  • Participating in team and process improvement meetings to refine workflows.
  • Coordinating with hospitals, clinics, and pharmacies to resolve clinical concerns.
  • Scheduling telephonic or video conference appointments with clinical pharmacists.
  • Assisting with referrals and authorization requests.
  • Supporting additional projects as assigned.

Our Ideal Candidate Possesses:

  • Active state Pharmacy Technician license.
  • National certification (PTCB or ExCPT) is highly preferred.
  • Strong ability to multitask and prioritize in a fast-paced environment.
  • Familiarity with medical terminology and medication names (brand and generic).
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft 365, Google Workspace, and MacBooks.
  • A team-oriented mindset with the ability to work independently.
  • Availability to work 40 hours per week.

Nice-to-Haves:

  • Experience with electronic medical records (EMRs).
  • Background in ambulatory care, particularly medication history documentation.
  • Previous experience in a high-volume healthcare setting.
  • Bilingual proficiency is a plus.

Remote Work Requirements:

  • A private, secure workspace that ensures patient information privacy.
  • A stable high-speed internet connection to support telephonic work.

Schedule:

  • 6:00am-2:30pm PST
  • 9:30am-6:30pm PST

Pay: $20-$22/hour
