Job description

Are you passionate about making a difference in the healthcare field from the comfort of home? We're seeking a dedicated Care Manager to contribute to a tight knit team making meaningful changes!

This is a temporary position from September 2024 to the end of the year, great opportunity to gain some experience and make connections in the field!

M-F 8am-5pm MST

FULLY REMOTE but must live in the state of Colorado, no exceptions.

$28.00-32.00 Per hour, equipment provided

What You'll Do:

  • Manage work queues to ensure timely member support and meet performance metrics.
  • Coordinate outreach efforts for members, connecting them with essential behavioral health resources.
  • Complete health assessments, establish person-centered care plans, and provide care coordination services.
  • Educate members on state plan benefits, programs, and services, and monitor their satisfaction.
  • Collaborate with interdisciplinary teams to ensure seamless care delivery and quality services.
  • Maintain accurate documentation, uphold ethical standards, and participate in team meetings and training.

What We're Looking For:

  • Prefer Bachelor's degree in social work or related, or experience.
  • Experience in Medicaid, healthcare, and care coordination.
  • Bilingual in English/Spanish is highly desired, not required
