Job description

*** NOW HIRING ***

Position: UAS Assessment Nurse (hybrid)

Schedule: Hybrid, 2-3 days remote, 1-2 days in field (travel around Bronx, NY)

Hours: 8:30am - 5:30pm, Monday - Friday

Overview:

Join our team as a UAS Assessment Nurse and play a crucial role in assessing members' healthcare needs and plan eligibility across downstate New York. This position offers a unique blend of remote work flexibility and field visits, providing an opportunity to make a meaningful impact in the community.

Responsibilities:

  • Conduct field assessment visits in the Bronx and surrounding areas, approximately 2-3 visits daily.
  • Complete documentation required by the Department of Health (DOH) to determine members' enrollment, service needs, and plan eligibility.
  • Coordinate with the Care Management Team (CMT) to ensure efficient and effective operations of long-term care plans.
  • Perform remote documentation and paperwork after field visits from home.
  • Utilize provided laptop and equipment for job-related tasks.

Qualifications and Requirements:

  • Active NY RN license (unrestricted)
  • Active NY Driver's License or NY State ID
  • new grads welcome
  • UAS experience NOT required
