Registered Nurse - Compassionate In-Home and Office Care

This role is essential in providing high-quality care for our patients, developing tailored care plans, and ensuring every patient receives the care and attention they need.

Position Overview In this role, you will work primarily in patients' homes, with occasional office visits as requested by management. This role offers flexibility with weekday hours between 8 am and 6 pm, averaging 30-35 hours per week.

Key Responsibilities

• Respond quickly to urgent patient needs with emergency home visits when necessary
• Collaborate with the care team to coordinate interventions and maintain clear communication
• Develop individualized care plans, assisting the Care Manager to support patients in reaching their goals
• Ensure full compliance with healthcare regulations and company policies
• Engage with patients by phone to provide support and address concerns, ensuring a positive experience
• Work closely with caregivers, healthcare professionals, and Case Management Agencies to deliver patient-centered care

Qualifications (Must Have)

• Registered Nurse (RN) license in Colorado
• Minimum one year of clinical experience, ideally with geriatric or disabled populations
• Reliable transportation for in-home visits

Preferred Skills (Nice to Have)

• Background in home care or experience in startup environments
• Familiarity with CRM systems
• Comfortable with technology and adaptable to a fast-paced environment
