Job Description

Position Summary:
The Registered Nurse Assessment Coordinator (RNAC) is responsible for delivering care to residents using the nursing process of assessment, planning, intervention, implementation, and evaluation. Under the direction of the attending physician, the RNAC ensures residents receive emotional, physical, and psychological support in a safe environment while maintaining all professional nursing standards. This role is critical for coordinating and overseeing the completion of assessments and care plans for residents, ensuring accurate documentation for reimbursement, and working closely with interdisciplinary teams.

Responsibilities/Job Duties:

  • Coordinate completion of Minimum Data Set (MDS) for Nursing Home Resident Assessment and Care Screening.
  • Update department policies and procedures annually.
  • Lead weekly Interdisciplinary Care Planning meetings and formulate care plans.
  • Review and update quarterly plans for change in condition.
  • Coordinate Medicare billing and perform medical record audits.
  • Chair the Resident Care Assessment Sub-committee and prepare reports for the Quality Assurance meeting.
  • Maintain accurate and up-to-date documentation for residents' clinical records and care plans.
  • Meet with residents and/or families quarterly to review care plans.
  • Other duties as assigned.

Minimum Education and Experience Qualification Requirements:

  • High school diploma or equivalent.
  • Graduate of an accredited school of nursing.
  • Valid WV state licensure as a Registered Nurse.
  • Current AHA Health Care Provider Training (BCLS).
  • Proven experience in a skilled nursing facility.
  • Proven knowledge and experience with MDS assessments and coding.
