Job description

Overview: We are seeking a dedicated and compassionate Registered Nurse (RN) to join our PACE program team in Aurora, CO. This role involves completing home visits for participants, working closely with providers to create and implement care plans, and delivering skilled care in participants' homes.

Schedule: Sunday to Thursday, 8 hours per day (flexible start/end time between 7 AM - 7 PM)

Pay: $45.25 - $50.00/hour

Responsibilities:

  • Perform home visits for PACE participants, following a clear daily schedule via the Access Care app on a provided iPhone.
  • Use a provided laptop for charting and documentation in EPIC (EMR).
  • Collaborate with providers to create and execute individualized care plans.
  • Conduct skilled care visits, including vitals, blood sugar monitoring, medication and IV management, lab draws, wound care, and medication administration.
  • Perform clinical assessments and update care plans as necessary, with some assessments required every 6 months and others annually.
  • Conduct follow-up visits within 24 hours after a participant's fall.
  • Manage a caseload of 5-10 patients per day, with each appointment typically lasting about 30 minutes.
  • Conduct 30-day reviews on CNAs and 90-day reviews on LPNs, involving home visits to evaluate their performance.
  • Start and end the workday from home, with occasional trips to the Day Center to gather supplies.
  • Home visits will be within a 10-20 mile radius of the Aurora Day Center. Mileage reimbursement is provided at 65.5 cents per mile.

Qualifications:

  • Minimum of 1 year of experience working with the elderly or frail population in homecare, assisted living, hospice, short-term/long-term care facilities, or similar settings.
  • Current RN license in the state of Colorado.
  • Strong clinical assessment and care planning skills.
  • Ability to work independently and manage time effectively.
  • Excellent communication and interpersonal skills.
