Job description

Role:

  • Given an iPhone - there is an App installed (Access Care) that will give them a clear layout of their daily schedules (what particpants they need to see, and what tasks need to be done during the visit)
  • Will have a laptop for charting (EMR = EPIC)
  • Working direclty with Providers on creating and implementing care plans, then will be responsible for caring those care plans out
  • Will be conducting skilled care visits - duties can include vitals, blood sugar, medication management, IV management (antibiotics, incilin, etc), lab draws, wound care, medication administration, etc
  • Conducting clinical assessments on participants (some are required every 6 months, some will be annually) - Making changes to care plans as necessary
  • RN's are required to conduct participant visit 24 hours after a fall occurs
  • 5-10 patients/day, typically about 30min appointments
  • Will conduct 30 day reviews on CNA's, and 90 day reviews on LPN's- This will consist of going to the participants homes & asking how the CNA/LPN is performing during home-visits to evaluate performance
  • Will start/end their days at home
  • Will be reimbursed 65.5 cents/mile

  • Monday - Friday 7:00 AM - 3:00 PM

Rates:

  • Pay: $45.25 - $50.00/hr

Dress code:

  • Scrubs - Any color/pattern

Must Have:

  • Drivers License/auto insurance
  • Minimum of one year experience working the frail or elderly
  • Active RN in CO
