Job description

The Registered Nurse (RN) will be an integral member of our Denver-based PACE program, responsible for conducting home visits to provide exceptional care to our participants. You will be equipped with a phone featuring an app for daily schedule management, and a laptop for charting. As an RN, you will collaborate closely with healthcare providers to develop and execute personalized care plans and deliver skilled care services to our participants.

Key Responsibilities:

  • Conduct home visits for participants within a 10-20 mile radius of the Day Center.
  • Efficiently manage daily schedules, ensuring participants receive necessary care and services.
  • Administer skilled care, which may include vital sign monitoring, blood sugar checks, medication management, IV care (antibiotics, insulin, etc.), lab draws, wound care, and medication administration.
  • Perform clinical assessments on participants, with some assessments occurring every 6 months and others annually, and adjust care plans as needed.
  • Respond to falls by conducting participant visits within 24 hours.
  • Conduct 30-day reviews on Certified Nursing Assistants (CNAs) and 90-day reviews on Licensed Practical Nurses (LPNs) to assess performance during home visits.
  • Collaborate with the Day Center to gather supplies for daily visits.
  • Start and end your workday from home.

Qualifications:

  • A minimum of 1 year of professional experience working with elderly and frail populations in settings such as home care, assisted living, hospice, short-term/long-term care facilities, etc.
  • Current Registered Nurse (RN) license in the state of Colorado.
  • Proficiency in using electronic medical record systems, preferably EPIC.
  • Strong clinical skills and the ability to perform skilled nursing tasks.
  • Excellent communication and interpersonal skills.
