Job description

Position: Registered Nurse (RN) - Family Medicine Department

Location: Codman Square Health Center

Position Overview: The Registered Nurse (RN) will provide patient care in the Family Medicine Department, seeing between 16-32 patients per day. Key responsibilities include triaging patients, managing the in-basket, performing wound care, administering injections/vaccinations, guiding Medical Assistants, and assisting providers.

Reporting Structure: The RN reports to the Nurse Supervisor and is responsible for the assessment, planning, implementation, and evaluation of patient care. They will work closely with Medical Assistants and Licensed Practical Nurses and perform charge nurse responsibilities.

Key Responsibilities:

  1. Nursing Assessment:

    • Triage and assess patients
    • Collect patient data, history, and vital signs
    • Review medical records
  2. Treatment Plan of Care:

    • Collaborate with patients, families, providers, and the care team to develop a treatment plan
  3. Implementation of Plan:

    • Carry out the individualized treatment plan for high-quality patient care
    • Implement provider orders and advocate for optimal care
    • Prepare and assist with medical exams
  4. Evaluation and Revision of Plan:

    • Evaluate and revise the treatment plan based on patient response
  5. Patient Safety:

    • Ensure service standards meet or exceed regulatory guidelines (JAHCO, DPH, MA Board of Nursing, etc.)
    • Apply standard precautions in patient care
  6. Worksite Safety:

    • Maintain a safe environment for patient care
  7. Supervision and Delegation:

    • Supervise and delegate tasks to Medical Assistants and LPNs
  8. Other Duties:

    • Perform other assigned duties and work at different locations if necessary

Professional Development: The RN will participate in care team huddles, clinical meetings, and quality improvement activities. They will seek learning opportunities, adapt best practices, and communicate openly about areas for improvement. The RN will also help mentor new staff and promote teamwork.
