Title: Registered Nurse

Summary: Enhance patient experience, improve access, and achieve better health outcomes for senior adults. This role involves direct patient care, education, and collaboration with providers and clinical staff in a Monday-Friday day shift schedule.

Day to Day:

• Perform daily patient care, including phone and in-person triage and education.

• Collaborate with Providers and Center Manager to enhance clinical performance.

• Monitor compliance with national and organizational care standards.

• Prepare for quality audits and respond to deficiencies.

• Educate patients, families, and staff on clinical conditions and care processes.

• Oversee clinical quality, patient safety, and staff training.

• Ensure compliance with clinical policies and procedures.

• Work closely with Medical Assistants to ensure smooth patient processes.

• Handle same-day appointments and educate patients on chronic condition management.

• Must be comfortable triaging patients over the phone and directing care appropriately.

Qualifications:

• AA/AS in Nursing required; BSN or Healthcare Administration preferred.

• Active RN license in the state of employment required.

• Minimum 5 years of direct nursing/medical experience.

• Experience in community-based clinics, public health, or geriatric care preferred.

• BLS certification required.
