Title: Registered Nurse
Summary: Enhance patient experience, improve access, and achieve better health outcomes for senior adults. This role involves direct patient care, education, and collaboration with providers and clinical staff in a Monday-Friday day shift schedule.
Day to Day:
Perform daily patient care, including phone and in-person triage and education.
Collaborate with Providers and Center Manager to enhance clinical performance.
Monitor compliance with national and organizational care standards.
Prepare for quality audits and respond to deficiencies.
Educate patients, families, and staff on clinical conditions and care processes.
Oversee clinical quality, patient safety, and staff training.
Ensure compliance with clinical policies and procedures.
Work closely with Medical Assistants to ensure smooth patient processes.
Handle same-day appointments and educate patients on chronic condition management.
Must be comfortable triaging patients over the phone and directing care appropriately.
Qualifications:
AA/AS in Nursing required; BSN or Healthcare Administration preferred.
Active RN license in the state of employment required.
Minimum 5 years of direct nursing/medical experience.
Experience in community-based clinics, public health, or geriatric care preferred.
BLS certification required.
