Job description

Role Scope:

We are seeking a dynamic, service-oriented dietitian who is passionate about supporting elderly persons in their journey. The Dietitian will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. The Dietitian will complete participant nutritional assessments and reassessments, screen and assess participants' nutritional status, and use pertinent data to plan and implement appropriate nutrition interventions. Additionally, the Dietitian will obtain participant food preferences, make dietary modifications as needed, and provide nutrition education to participants, caregivers, and staff. Our Sacramento PACE Dietitian will set the vision for our overall medical nutrition program for our participants.



Participant Care:

  • Develop and implement nutritional plans of care for participants that take into account social circumstances and medical needs, in collaboration with the PCP and interdisciplinary team.
  • Plan and execute follow-ups and reassessments for each participant, or as necessary if there are changes in participant status.
  • Assess participants' access to food and make referrals to the interdisciplinary team for meal preparation evaluation and grocery shopping needs.
  • Determine participants' need for and order liquid nutritional supplements accordingly.
  • Provide instruction to participants and caregivers regarding dietary regimens and nutritional requirements (home visits may be necessary).
  • Develop and implement nutrition care plans for high-risk participants, including medical nutrition therapy recommendations.
  • Monitor food intake, therapeutic diets, and special dietary modifications, and communicate information and concerns with the care team.


Administration & Program Development:

  • Conduct sanitation and safety checks for kitchen and kitchen equipment as needed.
  • Monitor adequacy of lunch and snack menus and note/make updates as needed.
  • Update the lunch list, supplement sheet, diet lists, and assisted dining room guide as needed.
  • Attend care planning sessions and report on the nutritional status of participants.
  • Work with the State Food Program as guided by state rules and regulations.
  • Serve as a subject matter expert for the development of dietary program standardization, evaluation, and team education.
  • Support quality programs and discipline-specific audits as a subject matter expert.


Required Qualifications:

  • Bachelor's degree in food science, nutrition, and/or dietetics.
  • Current medical terminology and the ability to complete written assessments and document participant care.
  • Certification by CDR as a registered dietitian.
  • A minimum of one year of experience working with the frail or elderly is required.


Preferred Qualifications:

  • 2+ years as a practicing dietitian.
  • A state-issued driver's license, personal transportation, and auto insurance as required by law.
