Job description

JOB SUMMARY

Under the supervision of the Site Team Leader (STL) the South Region PrEP Coordinator will provide general oversight of PrEP services in the south side/south suburban PrEP Clinics. The Coordinator will assist in the planning, development, implementation and evaluation of the south region PrEP program as well as the creation of service protocols. Will monitor outcomes and program's scopes and makes recommendations to insure the monthly, quarterly and yearly scopes are met. Conducts program evaluation to assess programmatic and procedural outcomes. Provide monthly quality assurance assessments and on a quarterly basis through scope numbers and direct observation. Prepare and summate progress reports to supervisors and leadership and monthly/quarterly reports collaboratively with Data Team members at each site.

The Coordinator will provide one on one HIV counseling, testing, PrEP assessments and PrEP referrals, which includes follow-up of positive results and the necessary referrals to a HIV Primary care clinic. Assesses group health and PrEP education needs on site and in the community; maintains up to date knowledge base of prevention interventions. Will act as the South Region Community Testing Coordinator, alongside the Regional HIV Testing Coordinator. Will plan and execute regional community-based HIV testing events, mobile HIV testing, pilot home-based HIV testing, PrEP outreach, assessments and referrals as needed and in any other events to PrEP eligible clients where PrEP services can be offer/needed and recruitment can be successful. Conducts community sexual health education sessions as related to HIV/STIs prevention and risk reduction, PrEP services assessment, referrals and initiations as needed in clinical settings. Works alongside the Site Team Leader to lead Community Health Worker-EIS/Outreach during testing in non-clinical settings. Works alongside the STL to oversee the ED Fast track HIV/STI/PrEP activities. This includes weekly monitoring reports positive STI referrals for HIV or PrEP linkage. Acts as Co lead for Domains 1 & 3. Work as a team member with the multidisciplinary teams of each site to assure/offer the best PrEP services in the communities we serve. Participate in all administrative meetings and staff development sessions. Work alongside the Data Team and Regional Director of Quality Management to evaluate the South Region PrEP programs. Works collaboratively with Leadership to ensure the provision of provider and clinical support staff to act as "PrEP champions". Work hours that include evenings and weekends. Attend meeting representing PrEP services, issues, activities, goals and outcomes. Develop and implement new activities that promote successful PrEP use.

SPECIFIC RESPONSIBILITIES

Principal Duties and Responsibilities:

  • Participates in the development, implementation and evaluation of protocols and procedures related to HIV/STI counseling and testing as well as PrEP education, assessments, and referrals for initiation in any of the sites where PrEP services is provided.

JOB DESCRIPTION

  • Assigns PrEP Navigators and Peer Educators to specific and PrEP clinic sessions in the different sites.
  • Provides training and daily supervision for PrEP Navigators and Peers and conducts annual performance evaluation with individual staff.
  • Serve as a PrEP resource to the different sites in the south side to staff and patients.
  • Maintains best prevention/PrEP practices and shares knowledge of procedure in the different sites.
  • Participate as a team member assisting colleagues and staff as necessary to attend training sessions and institutional meetings.
  • Serve as a PrEP Navigator in PrEP clinic sessions in the different sites.
  • Coordinates staff meetings and continuing education programs to update PrEP Navigation, Peer educators and clinic staff on new or revised procedures while maintaining PrEP services coverage.
  • Participates in all administrative, Domain 1 & 3 and staff development meetings, representing PrEP, community-based HIV testing services issues, activities, goals and outcomes.
  • Assist in review and assessment of the current PrEP services/procedures in the different sites and in the implementation of the best practices.
  • Create new procedures at site where PrEP services are not efficient or needs improvement.
  • Work with the PCMH Director and the multidisciplinary team of each site to improve PrEP services and better utilize existing resources.
  • Monitor procedures and outcomes and make recommendations
  • Monitor program monthly and quarterly scopes,
  • Produce reports for supervisors, and administration and funders in a timely matter.
  • Works together with the other Regional Coordinators (Regional Community Testing coordinator, PrEP West Region) in the implementation of PrEP and Community based HIV
  • Testing services and serve as a backup.

QUALIFICATIONS

  • Minimum of a high school diploma or equivalent required. Bachelor's degree in a health-related field, education, psychology, social science preferred. A minimum of three years prior experience in HIV testing, health education and PrEP counseling required. With 1 year of Managerial experience.
  • Bilingual preferred (Spanish-English). Perform as a team player and demonstrate ability to work on a multicultural, multidisciplinary team. Strong communication (oral and written), organizational,
  • excellent interpersonal and problem-solving skills. Availability to work evenings and weekends as
  • needed. Access to a vehicle or transportation required. Ability to meet deadlines. Stamina and
  • flexibility to perform and feel comfortable in a fast-paced, sometimes chaotic clinical environment.
