Regional Community PrEP Navigator

Job Description:
We are seeking a passionate Regional Community PrEP Navigator to provide PrEP navigation and referral services at community clinics with high HIV and STI prevalence. This role involves HIV/STI counseling, education, outreach, and patient navigation to connect individuals with PrEP services and other healthcare resources.

Key Responsibilities:

• Provide HIV/STI education, testing, and risk reduction counseling
• Conduct community outreach and recruit individuals for PrEP services
• Assist patients with PrEP enrollment, insurance, and medication assistance programs
• Document patient interactions in Cerner, CareWare, or PROVIDE
• Collaborate with healthcare teams to ensure seamless PrEP linkage and follow-ups

Qualifications:

• 1-3 years of experience in HIV prevention, PrEP navigation, or health education
• Strong communication and outreach skills
• Bilingual (Spanish-English) preferred
• Ability to work evenings and weekends as needed
• Valid driver's license and insurance required
