Job description

The Referral Coordinator is responsible for evaluating and reviewing referrals of homeless clients from hospitals or other agencies contracted with Illumination Foundation Recuperative Care Center to ensure potential clients meet admission criteria for the program. Coordinates with hospital/IPA/MCO contact by managing the discharge of homeless clients to assist in the successful transition back into the community and performs admin duties that abide by Recuperative care contract verbiage.

Referral Process/ 2 hrs. maximum Incoming referrals arrived via fax or e-mail, anytime Monday through Sunday 24 hours a day

- Review incoming hospital/MCO/IPA client referrals under the preferred 2-hour turn-around time.

- Assessing if additional supporting documents are needed.

- Calling referral Case Manager (CM) contact to request documents to be faxed prior to review. (i.e. H and P, Physical Therapy/ Occupational Therapy notes, Psych evaluation, med lists)

- Gather preliminary referral information and input on 'IF Recuperative and Recovery Intake Exit Google Document", Pending referral section.
