Job Overview:

We are seeking dedicated and compassionate Bilingual Community Health Workers (Spanish/English) to provide personalized care coordination and patient advocacy to medically underserved communities. In this role, you will work with vulnerable populations facing barriers to care, ensuring they receive the medical, behavioral, and social services they need.

As a key liaison between patients and healthcare systems, you will support individuals by navigating healthcare programs, connecting them with local resources, and addressing social determinants of health (SDOH). This role requires field-based engagement, case management, and community outreach, allowing for both virtual and in-person interaction with patients.

Key Responsibilities:

• Manage a caseload of local members, offering a mix of virtual and in-person case management services.
• Conduct comprehensive health assessments to evaluate physical, behavioral, and social needs.
• Develop and execute individualized care plans to address barriers to healthcare access and social needs.
• Collaborate with clinical teams, community organizations, and social service providers to ensure coordinated patient care.
• Facilitate appointments, transportation, and referrals for medical, mental health, and essential services (e.g., food programs, housing assistance).
• Engage in community outreach, including participating in health fairs, public education efforts, and resource navigation events.
• Provide culturally competent health education and guidance on chronic disease prevention, mental wellness, and healthcare utilization.
• Advocate for patients by helping them navigate complex healthcare systems, apply for benefits, and address language barriers.
• Accurately document patient interactions, assessments, and interventions in electronic health record (EHR) systems while maintaining HIPAA compliance.

Qualifications:

• Bilingual in Spanish and English (written and verbal) required.
• 1-3 years of experience in healthcare navigation, case management, social work, or community outreach.
• Experience in Enhanced Care Management (ECM) or similar care coordination programs preferred.
• Strong cultural competency and relationship-building skills to establish trust within the Latino community.
• Ability to work independently in a hybrid role, balancing virtual case management with in-person visits.
• Valid driver's license and reliable transportation for fieldwork and community engagement.
• Must be located within one of the hiring California regions (Monterey, Merced, or Santa Cruz).

Preferred Skills & Experience:

• Experience documenting patient care in electronic health record (EHR) systems.
• Familiarity with Medi-Cal, Medicaid, and social service enrollment processes.
• Training in motivational interviewing, trauma-informed care, and nonviolent crisis de-escalation (preferred but not required).
• Background in public health, social work, or health education is a plus.

• Competitive hourly rate starting at $26/hr.
