Employment Type: Full-Time, Hybrid (Work from Home & In-Field)
We are seeking dedicated and compassionate Bilingual Community Health Workers (Spanish/English) to provide personalized care coordination and patient advocacy to medically underserved communities. In this role, you will work with vulnerable populations facing barriers to care, ensuring they receive the medical, behavioral, and social services they need.
As a key liaison between patients and healthcare systems, you will support individuals by navigating healthcare programs, connecting them with local resources, and addressing social determinants of health (SDOH). This role requires field-based engagement, case management, and community outreach, allowing for both virtual and in-person interaction with patients.
