Promoter de Salud - Bilingual Community Health Worker
Location: Various regions in California including Placerville, Markleville, Fresno, Bakersfield, San Francisco, Santa Clara, Sacramento, Los Angeles, and more.
Employment Type: Full-Time, Hybrid (Work from Home & In-Field)
Job Overview: We are seeking dedicated and compassionate Bilingual Community Health Workers (Spanish/English) to engage with patients both virtually and in the field. As a key liaison, you will support patient populations in your designated region by providing essential health navigation services, promoting mental, physical, and social wellness, and connecting them with local healthcare resources.
Key Responsibilities:
Manage a caseload of up to 50 patients, providing virtual and in-person case management services.
Conduct initial health assessments to identify physical, behavioral, and social health needs.
Develop and implement personalized care plans addressing social determinants of health.
Collaborate with clinical teams to connect patients to necessary medical, mental health, and community resources.
Facilitate patient appointments, transportation, and access to essential services such as food programs.
Participate in community outreach events to promote health services and engage with the local population.
Qualifications:
Bilingual in Spanish and English (written and verbal).
Strong experience in enhanced care management, healthcare case management/social work, or related fields.
Strong communication skills and cultural competency to build trust within the community.
Ability to work independently in the field and virtually.
Valid driver's license and reliable transportation.
Compensation and Benefits:
Competitive hourly rate starting at $26/hr.
$0.67 per mile reimbursement for travel.
$1,000 home office stipend distributed over initial 6-month period.
Company-provided laptop.
Bonus incentives based on performance and patient engagement.
