Job description

Problem-Solving, also known as "Diversion" or "Rapid Resolution", is a crisis response philosophy and approach that focuses on supporting individuals and families experiencing a housing crisis to quickly identify and access alternative housing resources outside of the homeless crisis response system. Through active listening and creative conversations, Problem Solving Specialists facilitate this process and assist households in maintaining their current housing or identifying an immediate and safe housing alternative within their own social network, such as with family or friends.

  • Provide mediation between households and property owners/family/friends to assist in resolving housing crisis
  • Partner with legal service providers to resolve housing crisis. * Process and accept referrals from funders
  • Meet with assigned guests a minimum of 1x/week to support with progression towards goals identified by guest in their Housing Service Plan
  • In collaboration with supervisor or MSW/LCSW staff, conduct intake and individualized needs assessment for all guests and assist team by collaborating with guests to develop Individualized Service Plans (ISP) that address barriers to obtaining services, housing, and appropriate health care needs

WHAT YOU BRING

  • Demonstrated knowledge and or experience with case management techniques, including critical time intervention, harm reduction strategies, crisis intervention techniques and motivational interviewing
  • Strong belief in a growth mindset, self-determination, and psychological resilience
  • .Ability to execute and maintain records and documentation in compliance with all state, federal, and local guidelines and laws and standards set forth by the program contract and PATH guidelines including maintaining confidentiality and security.
  • Ability to maintain and execute confidential information according to HIPAA standards
  • Possess a high level of tolerance and understanding for individuals who present for service

PREFERRED QUALIFICATIONS
An excellent candidate for this role will:

  • Two (2) years of experience working with individuals or families experiencing or at imminent risk of homelessness or another related field
  • HMIS Experience.
  • Demonstrated knowledge of and/or experience with advanced and evidence-based interventions including, but not limited to: - Harm Reduction strategies - Motivational Interviewing - Housing First Practices - Non-violent Crisis Intervention Techniques - Critical Time Intervention - Trauma Informed Care

MINIMUM QUALIFICATIONS
To be considered for this position, we require all candidates:

  • All levels of education and experience welcome.
  • Bilingual and able to read, write and speak Spanish proficiently

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver's license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage
