Job description

Job Title: Physical Therapist (PACE Program)
Salary: $120,000 - $135,000 annually, with potential for an annual performance bonus of up to $7,000
Schedule: Monday - Friday, 8:00 AM - 5:00 PM
Location: 100% Onsite (No Hybrid/Remote)

We are a leading healthcare provider dedicated to offering fully integrated health and social care to older adults on Medicare or Medicaid who want to maintain their independence and live in their own homes and communities. We operate through the Program of All-Inclusive Care for the Elderly (PACE) in collaboration with local community partners, providing comprehensive care to meet the unique needs of older adults.

Position Overview:
We are seeking a passionate and experienced Physical Therapist to join our interdisciplinary care team. This role is essential in helping older adults remain independent by providing high-quality rehabilitation services. The ideal candidate will have a strong commitment to enhancing the quality of life for seniors, experience working with Medicare and Medicaid populations, and a desire to work in a collaborative environment.

Key Responsibilities:

  • Conduct thorough patient evaluations and develop individualized treatment plans to improve physical mobility and function.
  • Administer physical therapy treatments to older adults in the PACE program, focusing on enhancing independence and quality of life.
  • Collaborate with an interdisciplinary team, including physicians, social workers, and nurses, to ensure comprehensive care.
  • Educate participants and their families on therapeutic exercises, injury prevention, and home safety to promote long-term success.
  • Monitor and adjust treatment plans as needed, ensuring progress toward patient goals.
  • Maintain accurate and timely documentation in compliance with PACE program standards.
  • Participate in care plan meetings and contribute to the holistic care approach for participants.

Qualifications:

  • Licensed Physical Therapist (PT) in [State Name] with current, active certification.
  • 2+ years of experience in geriatrics, home health, or a similar setting, preferably with Medicare/Medicaid populations.
  • Strong knowledge of physical therapy principles and the ability to tailor treatment plans to meet the needs of older adults.
  • Excellent communication skills and ability to work collaboratively in a multidisciplinary team environment.
  • Passion for improving the quality of life for older adults and commitment to patient-centered care.
