Role Scope:
We are seeking a dynamic, service-oriented physical therapist who is passionate about supporting elderly persons in their journey. The physical therapist will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care.
Participant Care:
- Provide physical therapy evaluation and treatment on an acute, restorative, and maintenance level to program participants.
- Participate in the Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination.
- Assess condition, create treatment plans, and discuss the implementation of therapy with participants and caregivers.
- Make home visits as needed and document activities, including participant/caregiver response in the participant's chart.
- Educate participants and family members in proper care and use of various supports and exercise programs, in addition to the use of DME.
- Document therapy services provided and perform department record-keeping in keeping with documentation requirements.
Administration and Program Development:
- Assist in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants.
- Serve as a subject matter expert for the development of physical therapy program standardization, evaluation, and team education.
- Support quality programs and discipline-specific audits as a subject matter expert.
- Maintain the quality assurance, safety, environmental, and infection control programs.
- Perform data collection, prepare reports, and maintain necessary participant statistics.
- Attend all staff meetings and patient care conferences as scheduled.
Required Qualifications:
- Bachelor's Degree in Physical Therapy.
- Completion of a physical therapy program accredited by the American Physical Therapy Association (APTA), the Committee on Allied Health Education and Accreditation of the American Medical Association, or other equivalent organization approved by CMS.
- Active Physical Therapist license issued by California.
- A minimum of one year of experience working with the frail or elderly.
Preferred Qualifications:
- 2+ years as a practicing PT.
- Master's Degree (MSPT) or Doctorate of Physical Therapy (DPT).
- A state-issued driver's license, personal transportation, and auto insurance as required by law.
