We are hiring for 2 Phone Triage Nurses to join our team in Pittsburgh, PA. The roles are primarily in quality implementation. Overview

  • Role is phone triage more than anything else
  • Role is also patient education
  • Role is also case management in some ways - identifying areas for preventive care, social determinants of health
  • Company serves the underserved - diverse clientel
  • The company does a lot of transgender care - a LOT of teaching, showing somebody with diabetes to inject insulin, hormone injections for somebody transitioning gender, etc
  • EMR: Athena
  • Phone triage

-Organizational skills, prioritizing inbox messages based on urgency
-Patient education
-Case management skills - social work part, if somebody's water is getting shut off, etc

Day to day:

  • Cover the providers, work with MA's - may get pulled to do minor clinical work if needed (patient edu mostly)
  • Mostly triage, they get a ton of calls, make calls, patient education which they do in person and on the phone
  • Exp drawing blood - dealing w walk in clinics
  • Patient messages are made into "patient cases" sent to nurse inbox - they work through these. Could be anything, like:

    inquiries about new meds; "my leg hurts", whatever the case may be; help setting appointments as needed based on next steps form patient case

The Quality Implementation Nurse will facilitate the mission of the FQHC to ensure the delivery of quality patient care and coordination of supportive services within the health center. The Registered Nurse will provide direct clinical support to an assigned provider, function as an EMR champion, and implement a patient-centered approach to all care given whether face-to-face or telephonic. The individual will administer to the needs of the patients by following the scope of practice and standards of care accurately while demonstrating a broad, comprehensive knowledge base in the theory and practice of nursing. This position utilizes the nursing process to ensure quality and cost-effective health care to patients in all stages of the lifecycle. The Quality Implementation Nurse will work independently as well as part of a team and requires a working knowledge of clinical applications.

  • Provide and facilitate competent health care of patients and families
  • Patricipate in outreach events (these are voluntarily and would not occur until after contract period)
  • Competently telephone triage for ages 0-150 (e.g. remote assessment, medication management,community resource management, chronic disease education, plan-of-care reinforcement, discharge follow-up, etc.)
  • Initiate care coordination support within the scope of Pennsylvania nursing licensure when necessary
  • Use computers for accessing electronic health records for data retrieval, data entry, charting updates, etc.
  • Provide health and wellness education to individuals and groups
  • Provide support for initiatives designed to improve the quality of care or clinical outcomes
  • Assist providers with direct patient care and procedures
  • Provide in-office education for patients with newly diagnosed chronic disease
  • Administer immunizations and injectable medications to patients age 0-150
  • Provide face-to-face and telephonic care to patients as assigned and qualified to perform
  • Assess, review, and develop nursing plans to ensure maximum patient outcomes
  • Provide patient/family/caregiver teaching, education, training and counseling as needed relative to disease processes, health maintenance, ordered procedures, and counseling regarding medication and nutrition
  • Complete all patient documentation and ensure all patient records are current and complete
  • Provide direct care to patients as assigned and qualified to perform
  • Coordinate care for patients with chronic diseases
  • Assist the clerical and clinical teams with the coordination and scheduling of patients
  • Assist in the education and training of staff

Requirements

  • Nurse Practitioner License
  • Preferably some women's health/OBGYN experience
  • Spanish speaking is a huge plus
  • Open to new grads
  • Needs to be willing to work with kids - they will train that experience is not required
  • Athena experience is a huge plus
