Job description

Job Title: Pharmacist

Location: Torrance, CA

Shift: Monday - Friday, 9:00am - 5:30pm - both an On Call rotation and a weekend rotation required (every 6 weeks)

Pay: $68 - 85/hr depending on experience

Job Summary:
The Pharmacist is responsible for ensuring regulatory pharmacy compliance, patient safety, clinical patient management, and effective communication with patients, caregivers, and physicians. This role involves coordinating and supervising the delivery of pharmacy services to patients in a home setting, including compounding, quality improvement, dispensing, drug information, and clinical monitoring activities.

Reporting Relationship:
Reports to the Director of Pharmacy.

Scope of Supervision:
This position does not have direct supervisory responsibilities.

Key Responsibilities:

  1. Receive and evaluate physician orders (both written and verbal) for appropriateness, including dosage, drug selection, potential interactions, route of administration, and stability.
  2. Ensure accurate compounding and dispensing of pharmaceuticals in compliance with pharmacy protocols and physician instructions.
  3. Provide drug counseling and consultation services to patients, addressing concerns and complaints.
  4. Order and review laboratory tests to monitor therapy effectiveness and minimize toxicity risks, notifying physicians of abnormal results and documenting such communications.
  5. Ensure compliance with all applicable laws, regulations, and accreditation standards while implementing best practices in pharmacy operations.
  6. Maintain comprehensive and accurate patient pharmacy records, including height, weight, allergies, medication profiles, progress notes, care plans, and laboratory results.
  7. Communicate with medical professionals regarding patient status, medication responses, and any necessary adjustments to treatment plans.
  8. Consult physicians regarding alternative treatment options in cases of drug interactions, stability concerns, or inappropriate prescriptions.
  9. Coordinate with office staff to schedule deliveries, manage care logistics, quantify medication amounts, and set up infusion pumps as needed.
  10. Stay informed on pharmaceutical products by interacting with manufacturers' representatives as necessary.
  11. Monitor and maintain adequate inventory levels to ensure availability of necessary supplies.
  12. Implement and maintain systems for tracking lot and batch numbers to facilitate medication recalls.
  13. Manage pharmaceutical recalls in compliance with pharmacy policies.
  14. Assume on-call responsibilities per pharmacy guidelines.
  15. Assist in maintaining pharmacy security protocols.
  16. Report product incidents as required by pharmacy policy.
  17. Supervise pharmacy technicians and other department personnel.
  18. Participate in inspections and surveys conducted by authorized agencies.
  19. Contribute to the pharmacy's Performance Improvement Program as directed.
  20. Serve on pharmacy committees as needed.
  21. Engage in pharmacy-provided in-service education programs.
  22. Pursue continuing education to maintain licensure and enhance job performance.
  23. Report any instances of misconduct, suspicious activities, or unethical behavior to the Compliance Officer.
  24. Perform additional duties as assigned by the supervisor.
  25. Ensure compliance with all relevant regulatory agencies, including ACHC, the Board of Pharmacy, the Board of Nursing, Title 22 Home Health Guidelines, Medicare, the Infusion Nurses Society, and NHIA.

Minimum Qualifications:

  1. Must hold a current State of California Pharmacist License.
  2. Strong organizational skills to maintain accurate records consistently.
  3. Ability to evaluate options and make sound decisions efficiently.
  4. Knowledge of state and federal pharmacy laws, rules, and regulations.
  5. Proficiency in computer applications, including spreadsheets, word processing, internet research, and email.
  6. Experience with CPR+ is preferred.
  7. Ability to work independently and exercise good judgment.
  8. Strong organizational skills and ability to collaborate across interdisciplinary teams.
  9. Excellent oral and written communication skills, with the ability to explain technical concepts in layman's terms.
