Job Description

Responsibilities/Job Duties:

• Oversee medical and surgical patients to ensure timely and effective discharge planning.
• Support patients in identifying and utilizing community resources for post-discharge needs.
• Conduct hospital room visits and, in specific cases, virtual sessions for recently discharged patients.
• Manage a caseload of up to 25-30 patients at a time, with oversight for up to 100 beds.
• Collaborate during interdisciplinary rounds with providers, utilization review, case managers, and other social workers.
• Address social needs and advocate for patients in a hospital setting.

Minimum Education and Experience Qualification Requirements:

Required Qualifications:

• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
• Open to new graduates with internship experience in a hospital setting.
• Familiarity with local community resources in Baltimore.

Preferred Qualifications:

• Prior experience in a healthcare or hospital setting.
• Pediatric social work experience in Baltimore.
• Experience managing high patient volumes in a fast-paced environment.
