Job description

Job Title: Patient Navigator

Location: Santa Monica, CA 90404

Hours: Monday to Friday, 8:30 AM to 5:00 PM

Salary: $36 to $40 per hour (Depending on Experience)

We are seeking a compassionate and dynamic Patient Navigator to join one of healthcare's biggest organizations! As a Patient Navigator, you will play a crucial role in supporting patients with pelvic health issues, including acute UTIs and post-surgery rehabilitation. Your responsibilities will include assisting patients through their healthcare journey, providing empathetic support, and coordinating clinical activities within our Women's Health and Wellness Institute.

Responsibilities:

  • Serve as a vital link between patients and healthcare providers, guiding patients through their pelvic health issues with empathy and compassion.
  • Coordinate clinical activities associated with the Women's Health and Wellness Institute, including program development, meeting coordination, and service coordination.
  • Act as a resource for patients, providing information and support regarding holistic care, including physical, psychosocial, emotional, and cultural aspects.
  • Collaborate with physicians, office managers, and diagnostic services to ensure seamless patient care and program development.
  • Facilitate education sessions on women's health and wellness topics for patients and staff.
  • Maintain accurate records, track program metrics, and report on navigation services and outcomes.

Requirements:

  • Bachelor's degree in Social Work or a related field required.
  • Previous experience in a healthcare setting preferred, with knowledge of ambulatory care processes.
  • Strong communication skills, with the ability to address clinical details and sensitive questions with empathy.
  • Comfortable with phone communication and customer service-oriented.
  • Proficiency in Electronic Medical Records (EMR) systems preferred.
  • Ability to work effectively in a fast-paced environment, balancing multiple tasks and priorities.
  • Fluent in English, with additional language skills beneficial.

Additional Information:

  • Training: Initial onboarding provided by Hospital for the first 3 days, followed by team training.
  • Interview Process: Initial phone screening followed by an in-person interview.
  • Start Date: Immediate start available.
  • Duration: Permanent position, open due to recent expansion.
  • Feedback: Feedback provided within 24 hours of interviews.
