Job description

**this role is fully remote but you need to be commutable to NYC if needed**

The Care Management Support Coordinator provides assistance to the Care Manager and other staff in the resolution of member related issues. This includes:

  • Communicating with members and their families regarding information about MA, FIDA and MAP products and services.
  • Providing summary reports and resources for care coordination related activities.
  • Participating in non-clinical customer service and assessment for members enrolled in the Health Plans MA, MAP, FIDA plan in collaboration with the Care Management department and other healthcare professionals.

This position is very phone heavy! 80% of the day will be spent communicating with members, conducting assessments and providing education on plans / policies.

Schedule is 9am - 5pm, Monday - Friday (EST) - there is a chance to flex to different days or hours as needed!

Experience Required

  • 2 Years Experience in a Call Center focused Customer Service environment
  • Experience in healthcare organization preferred
  • Proficiency in MS Word, Excel, and data systems required
  • Ability to work Remote / From Home
  • Able to commute to NYC if needed
