Job Description

Responsibilities/ Job Duties:

  • Initiates outreach efforts to potential enrollees, providing information on Long-Term Services and Supports Care Plan options and the role of LTSS Care Coordinators.
  • Uses motivational interviewing techniques to engage and inform potential enrollees about the program and enrollment process.
  • Performs unscheduled home visits as needed, particularly if no contact is made within 90 days of the initial outreach.
  • Tracks and manages outreach follow-up communications and documentation.
  • Obtains participation forms from enrollees who agree to join the Long-Term Services and Supports Care Plan program to confirm their consent.
  • Schedules intake appointments for enrollees with Intake staff.
  • Maintains quality communication with enrollees, internal staff, and external partners (including ACOs and physicians).
  • Actively participates in training activities, including shadowing Care Coordinators and working with a mentor during orientation.
  • Documents all activities and outreach efforts in the electronic health record system (eHana).
  • Prepares and distributes informational packages for home visits with enrollees.
  • Regularly communicates with NRLP Program Coordinator and other NRLP staff about outreach status.
  • Manages, tracks, and distributes outreach materials such as brochures.

Minimum Education and Experience Qualification Requirements:

  • Minimum Education: Associate's degree
  • Experience: Minimum of one year of patient outreach/enrollment experience preferrably with a similar program required
  • Must reside in Massachusettes
  • Preferred Qualifications:
    • Bachelor's degree in a related field.
    • Bilingual in Spanish (highly preferred).
