Job description

PRINCIPAL MISSION:

Under the general supervision of the Department and in clinical collaboration with a physician, the nurse practitioner provides comprehensive health care, determines and delivers primary care, engages in health and mental health screening and disease prevention. Supervision, consultation and teaching/training maybe provided to nursing, counseling, and other health care professionals.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  1. Promote a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c).
  2. Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct.
  3. Makes Customer Service a priority, treating customers (patients, visitors, and co workers) in a professional manner exercising courtesy and tact. E
  4. ffectively communicates with patients and others about SBH services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in SBH in a timely fashion.
  5. Works in collaboration with the attending physician within the approved
  6. Protocols established for the Department of Hematology / Oncology.
  7. Perform and document comprehensive medical and psychosocial health histories and examinations.
  8. Gathers data, conducts initial history and physical examination, and formulates and implements treatment plans in collaboration with physician.
  9. Orders and interprets diagnostic studies related to the patient's plan of care and response to treatment.
  10. Provides follow-up care on an inpatient and outpatient basis.
  11. Initiates consultation with specialists as indicated
  12. Recognizes oncology-related problems and initiates interventions.
  13. Identifies treatment-related toxicities and initiates interventions.
  14. Demonstrates expertise in symptom management.
  15. Assesses ill patients and develops plan of care in collaboration with MD
  16. Monitors and adjusts patient medications and treatments according to protocols. 16. Diagnoses and treats common oncology associated illnesses.
  17. Manages telephone calls related to emergencies, symptom management, and psychosocial and family concerns.
  18. Provides continuity of care across settings (hospital, home, home health, skilled nursing, hospice and rehabilitation).
  19. Supervises chemotherapy administration inpatient and outpatient settings.
  20. Collaborates with physician and chemotherapy infusion nurse to schedule and administer chemotherpay
  21. Writes chemotherapy orders and evaluates protocols in collaboration with physician.
  22. Provides psychosocial counseling and support to patients and families and makes referrals to social work or community agencies as necessary.
  23. Performs various procedures: bone marrow biopsy and aspirate, removal of picc, administration of intrathecal chemotherapy.
  24. Evaluates effectiveness of integrative therapies and their use in complementing medical treatment.
  25. Demonstrates knowledge of cancer risk factors, diagnoses, disease processes, and treatment options.
  26. Evaluates patient intervention outcomes.
  27. Provides Genetic Pretest Counseling
  28. Coordinates discharge plans, follow up, home health, and outpatient prescriptions. 29. Assists with hospital rounds and management of hospitalized patients.
  29. Participates in daily huddles/conferences with team to facilitate communication and coordination of care.
  30. Maintains and improves professional competence through participation in professional activities.
  31. Participants in peer review and the review of protocols and procedures.
  32. Instructs patient and/or significant other following plan of care incorporating . age/developmental level.
  33. Provides patient education pertaining to disease process and treatment plan
  34. Verifies and documents the patient's/significant other level of understanding through return demonstration and/or verbalization.
  35. Uses appropriate patient identification mechanism.
  36. Follows OSHA standards and other Infection Control policies.
  37. Demonstrates an understanding of the legal aspects of documentation (i.e., timing, sequence, accuracy, names, titles, etc.)

39. Acts as patient advocate.

QUALIFICATIONS:

  1. Education and Experience:

Master of Science Degree in Nursing with an accredited Nurse Practitioner curriculum and three years of nursing experience.

OR

Graduation from an accredited school of professional nursing, and from a recognized college or university with a Bachelor's Degree in Nursing and Master's Degree in a health related field and certification as a Nurse Practitioner from an accredited program and, in addition, three years of nursing experience.

Two years' experience in Oncology

  1. Required Knowledge, Skills, Abilities and Attributes:

Thorough knowledge of current nursing techniques and procedures;

Knowledge of the New York State Practice Act, the American Nurses Association Standards and the Joint Commission of Accreditation of Hospital Standards, particularly as they apply to the Nurse Practitioner, knowledge of the principles of patient education; ability to collaborate with physicians and other health care professionals in the development of primary health care.

  2. Current valid license to practice nursing in New York State
  3. Certification as ANP, FNP or PNP with prescriptive privileges by New York State 3. ANA certification in Adult or Family Practice preferred
  4. ONC certification
  5. Chemotherapy certification

WORKING CONDITIONS:

Thirty seven and one half hours (37 ½) per week, with flexibility to successfully complete duties.

JOB HAZARDS:

Occasional disturbed or otherwise difficult patients.

Exposure to communicable diseases.

Needle sticks.
