Nurse Practitioner - Home Health

Are you passionate about delivering whole-family care? We're seeking a Family Nurse Practitioner to provide comprehensive primary, acute, chronic, and pediatric care to families in their homes across Cook County.

• Responsibilities/ Job Duties:
• Provide whole-family primary care through home visits and virtual modalities.
• Collaborate with an interdisciplinary team, including MDs, RNs, and LCSWs, for comprehensive care.
• Document care, manage tasks, and communicate using electronic health records and tools.
• Participate in huddles and case conferences to address physical health needs and set health goals.
• Diagnose, treat, prescribe medications, and educate patients on conditions and treatments.
• Minimum Education and Experience Requirements:
• Active Family Nurse Practitioner license in the state of Illinois in good standing
• Must have a driver's license, a car (mileage is reimbursed), auto insurance, and the ability to drive to the homes of patient families and carry portable healthcare supplies
• Prior primary care or urgent care experience preferred
• Schedule/ Shift:
• Monday - Friday 5x8s or 4x10s. One Saturday morning per month. Possible rotating call.
