We are seeking an experienced Nurse Practitioner to perform wellness assessments for a growing, value-based healthcare organization. This is a hybrid position, involving remote/work from home responsibilities along with in-home patient visits in the Burlington, VT area.

Nurse Practitioner - Job Overview:

You will start your day working from home. You are able to make your schedule in regards to how many in-home patient visits you set per day. You'll then be responsible for going out to patient's homes to conduct wellness visits. There is no direct patient care work required, only assessments. All other work including charting will be done remotely from your home. Mileage reimbursement will be provided for all patient in-home visits.

Patient Demographics: Medicare/Medicaid senior population. This organization partners with major health plans to provide these in-home services to seniors.

Nurse Practitioner - Schedule:

• Monday - Friday 8-5

Nurse Practitioner - Requirements:

• Current and valid State RN license
• Bachelor's degree in Nursing and Advanced Practice Nursing license
• Board Certification from a nationally recognized certifying body for Nurse Practitioners
• Unrestricted prescribing and dispensing privileges with DEA licensure
• Current driver's license and proof of insurance
