Position: Nurse Practitioner

Location: Philadelphia

Schedule: Monday to Friday, 8:30 AM - 4:30 PM


About the Role:

We are seeking a dedicated Nurse Practitioner to join our team and provide primary and urgent care services to adults and families within our FQHC Network. This role involves serving diverse and underserved populations, in line with our mission and quality standards. The Nurse Practitioner will report to the Medical Director.

Essential Duties and Responsibilities:

  • Provide primary and urgent care services to patients.
  • Adhere to state and federal regulations and collaborative physician agreements.
  • Manage acute and chronic health conditions using evidence-based protocols.
  • Conduct health screenings, immunizations, and disease prevention programs.
  • Advocate for advanced practice nurses and engage in professional activities.
  • Notify the immediate supervisor of significant issues.
  • Perform age-appropriate assessments and interventions for patients and families.
  • Educate patients and families, offer health counseling, and provide health advocacy.
  • Administer age-appropriate immunizations.
  • Collaborate with community health nurses on care and follow-up.
  • Participate in public health management and other health initiatives.
  • Maintain positive relationships with funding sources and community organizations.
  • Document all services in a timely manner.
  • Supervise staff as appropriate.
  • Act as a mandated reporter to both the State of Pennsylvania and the Joint Commission.
  • Perform other related tasks and duties as assigned.

Requirements:

  • Current PA NP license
  • 2 years of experience in family medicine, primary care, or pediatrics
  • Fully vaccinated
