Medix is hiring a Nurse Practitioner to in-home assessments in North Lynn, MA! The NP must be in person for the visits, but all documentation and administrative work can be done from home.

Job Overview:

The Nurse Practitioner will be doing in home assessments. The NP will be Responsible for 12-16 wellness visits per week. Each home visit takes about 1 hour. The Nurse Practitioner collaborates with the Medical Doctors at the site to provide comprehensive primary care to the participants.They will be evaluating acute and chronic care conditions in line with insurance and state expectations of all participants.

Responsibilities:

• Conducts initial examinations of all new participants and quarterly examinations thereafter, for the first year of enrollment.
• Evaluates participants for episodic visits, acute illnesses and follow-ups of hospitalizations.
• Makes nursing home visits in accordance with the state nursing home requirements and as needed.
• Makes home visits as needed acutely or in chronic situations when participants are home bound.
• Provides appropriate health care teaching to patients, their families and caregivers, as well as to other members of the Interdisciplinary Team.
• Completes appropriate, accurate, timely documentation, onsite and offsite.
• Ensures that duties are accomplished in a fiscally responsible manner adhering to established budgetary constraints.
• Ensures that all interactions with clients are conducted in a manner that adheres to the highest standards of care.

Schedule:

Full time, Monday - Friday 8am to 4pm with no weekend or holiday hours.

Additional Information:

• Must be a licensed NP in Massachusetts
• The NP will be documenting into ECW in between visits that they can do at home or elsewhere.
• The NP will be given a laptop, iPad for a live translator and a couple of monitors for documentation purposes at home.
• Training duration depends on the NP's ability but is typically 2-4 weeks where they will be doing a few days of virtual training and then shadowing another NP for in-home visits.
• NP will be given mileage reimbursement
• After training, NPs will have the ability to make their own schedule (ideally a few visits/day but can stack them up if needed)
