Job description

Position: Wound Care Nurse Practitioner

Location: Lexington, Kentucky

Hours: FLEXIBLE: Monday - Friday, No Weekends

Pay: $120k - $140k depending on experience

A reputable national healthcare organization is looking to hire a Wound Care Nurse Practitioner to join their rapidly growing team!

Key Responsibilities:

  • Conduct thorough assessments of wounds to determine treatment plans and progress.
  • Utilize the molecular machine for accurate wound measurements and digital photography with integrated rulers.
  • Implement standard wound care practices, including debridement (both topical and surgical) and application of topical debriding agents to promote healing.
  • Administer cellular tissue products on open wounds as prescribed.
  • Take and maintain detailed photographic records of wound progress.
  • Collaborate effectively with home health agencies and SNFs to ensure continuity of care across patient visits.
  • Provide ongoing care by making regular rounds to the same SNFs and patient homes, maintaining consistency and rapport with patients.
  • Manage an average caseload of 20 patients per day across SNFs and patient homes.
  • Allocate time efficiently, spending one full day per week in SNFs and balancing the remaining days between SNFs and patient homes in designated areas.
  • Navigate between different locations within a manageable radius, typically within 5 miles, with mileage reimbursement provided.
  • Exercise discretion in patient selection and location assignments to prioritize safety and optimal care outcomes.
  • Work collaboratively with a Licensed Vocational Nurse (LVN) during all patient interactions to ensure comprehensive and compassionate care delivery.

Qualifications:

  • Active Kentucky Nurse Practitioner License
  • Experience working in a Skilled Nursing Facility (SNF)
  • Wound care, specifically surgical debridement
