Job description

The Nurse Care Manager is responsible for coordinating and managing comprehensive patient care within a multidisciplinary team. This role is focused on ensuring high-quality, patient-centered care while optimizing health outcomes. The Nurse Care Manager serves as a liaison between patients, healthcare providers, and support services, providing education, guidance, and advocacy.

Key Responsibilities:

  • Care Coordination: Develop and implement individualized care plans for patients, ensuring a smooth transition across various stages of care (hospital, home, rehabilitation).
  • Patient Advocacy: Act as a primary advocate for patients, ensuring their needs are met and their voices are heard in the care process.
  • Collaboration: Work closely with physicians, specialists, social workers, and other healthcare professionals to deliver comprehensive care.
  • Patient Education: Educate patients and their families on health conditions, treatments, and care plans, empowering them to manage their health effectively.
  • Chronic Disease Management: Monitor and manage patients with chronic conditions, providing ongoing support and adjusting care plans as necessary.
  • Utilization Review: Assess and evaluate the appropriateness of care provided, ensuring efficient use of resources and adherence to best practices.
  • Documentation: Maintain accurate and up-to-date patient records, including care plans, progress notes, and communication with other healthcare providers.
  • Quality Improvement: Participate in quality improvement initiatives aimed at enhancing patient care, reducing readmission rates, and improving overall patient outcomes.
  • Compliance: Ensure compliance with all regulatory requirements, hospital policies, and ethical standards.
  • Crisis Intervention: Provide timely intervention and support in emergency situations or when care needs escalate.
  • Resource Management: Identify and connect patients with community resources and support services, such as home health care, rehabilitation, or financial assistance programs.

Qualifications:

  • Education: Bachelor's Degree in Nursing (BSN) required.
  • Licensure: Active Registered Nurse (RN) license in NY.
  • Experience: Minimum of 3-5 years of clinical nursing experience, with at least 2 years in a case management or care coordination role.
  • Skills:
    • Strong clinical assessment and critical thinking skills.
    • Excellent communication and interpersonal abilities.
    • Organizational skills with attention to detail.
    • Ability to work collaboratively in a team-oriented environment.
    • Proficient in electronic health records (EHR) systems and care management software.
