Must Have:

• Active RN license in NY state
• Home Health experience within the last 15 years
• New York State ID or Driver's License
• Tech savvy with computer/technology

Schedule: Mon-Fri 8am-5pm

• Average assessments - 2.5 hours
• Remote Days: 3 assessments per day
• In Person Days: 2 assessments per day
• Sometimes a Clinician will go in with a Nurse etc.

Job Duties:

• Ability to conduct in-home health assessments throughout the assigned region in the state of New York.
• Ability to review health status questions and make evaluations and determinations based on UAS criteria.
• Computer literacy including ability to navigate Web-based applications, Microsoft Outlook & Microsoft Word; ability to navigate and conduct assessments via Zoom.
• Knowledge of the theory, principles, and practices of general professional nursing; knowledge of state and agency laws and regulations governing general professional nursing practices
• Excellent organizational, interpersonal, written, and verbal communication skills; ability to communicate and work effectively with multi-generational consumers.
• Ability to demonstrate sensitivity to the concerns of others.
• Ability to perform comfortably in a fast-paced, deadline-oriented work environment; ability to successfully execute many complex tasks simultaneously; and a strong ability to work independently
