Job Title: Social Worker (LMSW/LCSW) - Med/Surg Discharge Planner

Pay Rates: Based on years of experience + interview! (Sign on bonus as well)

• LMSW: $33 - $37/hr
• LCSW: $38 - $42/hr

Job Overview:

We are seeking dedicated and compassionate Social Workers to join our dynamic team at a large, resource-rich hospital in Baltimore. In this role, you will manage a caseload of 18 patients/beds, performing interdisciplinary rounds with physicians and RN Case Managers daily at 10 am. Your primary responsibility will be discharge planning, covering specific beds across the entire hospital as a Med/Surg Discharge Planner.

Key Responsibilities:

• Manage a caseload of 18 patients/beds
• Participate in daily interdisciplinary rounds with physicians and RN Case Managers
• Develop and implement discharge plans for patients
• Cover various departments within the hospital, focusing on Med/Surg patients
• Collaborate with a robust team to ensure seamless patient care and discharge processes

Must-Have Skills/Qualifications:

• • LMSW/LCSW MD license
  • Medical experiecne 1+ years within a healthcare field
  • Hopsital/clinical/acute care experience preferred
  • Discharge planning preferred

Schedule/Shift:

• Monday to Friday: 8 am - 5 pm
• On-Call: After hours, 2x a year from 4:30 pm - 10 pm (remote capable)
• On-Call Rotation: One full week (7 days)
• Weekend Work: 2 weekend days a month (non-consecutive), with a compensatory day off during the week to maintain a 40-hour workweek
