Job description

Medical Assistant / Care Navigator - HYBRID

Job Description:
- Medical assistant / care navigator
- Healthcare organization committed to providing medical and mental health care to patients in Assisted Living Facilities (ALFs) and Senior Living communities
- Administrative tasks (no direct patient care) - referrals, patient communication, setting up PT, etc.
- The Care Navigator will work closely with patients, caregivers, healthcare providers, and community staff to ensure
comprehensive, personalized coordination and care management for both chronic and mental health conditions (specifically dementia). This role is crucial in managing the care of patients with chronic conditions, improving patient outcomes, and ensuring continuity of care. They will work under the direction of our Licensed Clinical Social Workers and Providers.

Schedule:
Monday - Friday, approx 8a - 5p (flexibility)

Location:
HYBRID - Approx 50% remote, 50% at one of the facilities in Polk County (Bartow, Haines City, Lake Wales)
One location is home base - others will reimburse for mileage

Pay: $21 - 23/hr based on experience
Contract to hire, 800 probationary period then conversion to FTE
