Key Responsibilities:

• Clinical Excellence: Utilize your solid clinical skills to provide top-notch care.
• Hands-On Patient Care: Perform triaging, take vitals, conduct phlebotomy, and perform EKGs.
• Tech-Savvy Support: Assist with technology, including Zoom, Slack, laptop usage, and Elation EMR.
• Flexible and Mobile: Comfortable traveling and working in the field, providing in-home care.
• People-Oriented: Be a flexible, personable, and approachable team member.

Schedule/Shift:

• Monday to Friday, 8 AM - 5 PM
• Field Work: 4 days in the field (mobile clinic or in-home care). The homes are thoroughly vetted for safety.
• Manageable Caseload: 5 or fewer visits daily, sometimes with clinicians, sometimes solo.
• Patient Focus: Provide complex care for older adults with chronic conditions.

Must-Have Skills/Qualifications:

1. Solid clinical skills with experience in triaging, vitals, phlebotomy, and EKGs.
2. Technologically adept and willing to assist with various tech platforms (Zoom, Slack, laptops, Elation EMR).
3. Comfortable with travel and performing work in the field.
4. Flexible, with a strong ability to connect with people and provide empathetic care.
