Job description

LVN Case Manager

  • M-F, 8am-5pm

RESPONSIBILITIES:

  • Possesses or acquires basic knowledge and understanding of HHA regulations, insurance, authorizations, coding and documentation requirements (including Medicare, OASIS)
  • Manages a patient caseload as assigned and assists with other patient's needs when covering for other Case Managers
  • Oversees coordination of patient care needs determined through case conferences in collaboration with field staff, other departments and outside agencies and services as required
  • Ensures compliant documentation of all Patient Care Services in all settings, through case review and audits. Communicates deficiencies to appropriate staff and follows-up on corrections and completions within required timelines
  • Verifies or participates in provision of adequate after-hours on-call nursing support
  • Consults with and provides communication between physicians and staff in matters relating to patient services
  • Addresses the age specific and cultural needs of the population served
  • Directs, supervises, as requested, care provided in the Ambulatory Infusion Centers
  • Performs skilled nursing in the Ambulatory Infusion Center and in the patient's home as needed. see 718A
  • Assumes, as necessary or in the absence of Clinical Scheduler, Clinical Support or Authorization personnel, responsibility for those activities as they impact immediate patient need
  • Participates, as requested, in the review, analysis, and evaluation of effectiveness of Continuous Performance Improvement Program
  • Identifying and reporting, to Supervisor, staff skills or limitations
  • Providing documentation as needed or requested
  • Scheduling, with Supervisor's approval, mentors/proctors
  • Making patient visits to evaluate patient care situation
  • Assists with evaluation of nursing staff competencies through case review, case conferences, feedback and peer evaluations
