Job Posting: Licensed Practical Nurse (LPN) - Primary Care Team

Schedule: Monday through Friday, 8:00 AM to 4:30 PM
Location: Westwood, MA
Employment Type: Full-Time

Overview:
We are looking for a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care

Team. As the third LPN on the team, you'll play a vital role in delivering high-quality patient care, working closely with providers, and supporting seamless clinic operations. This position offers a dynamic mix of responsibilities, with 75% indirect patient care and 25% direct patient care.

Key Responsibilities:

  • Utilize EPIC, our electronic health records system, to manage "inbaskets," coordinate care, and assist providers.
  • Screen patient calls to assess the urgency of medical treatment.
  • Perform patient intake, including obtaining and documenting vital signs and pertinent health information such as temperature, pulse, blood pressure, height, and weight.
  • Conduct post-operative follow-up phone calls within 24 hours after surgical appointments.
  • Document all patient interactions, lab results, and prescription refills accurately in EPIC.
  • Collaborate with the Nursing Director and Practice Manager/Administrator to ensure clinical oversight and operational efficiency.

Qualifications:

  • Active LPN license
  • Experience in a clinical setting, with proficiency in electronic health record systems (preferably EPIC).
  • Strong communication and organizational skills.
  • Ability to work effectively in a collaborative, fast-paced environment.
