Job description

Role:

-This LPN will be completing home visits for participants in their PACE program for their allocated location
-Each LPN will be given an iPhone - There is an App installed (Access Care) that will give a clear layout of their daily schedules (what participants they need to see, and what tasks need to be done during the visit)
LPN's will also receive a laptop for charting (EMR = EPIC)
-Will be directly following the plan of care created by the RN
-Will be conducting skilled care visits - Duties can include vitals, blood sugar, medication management, IV management (antibiotics, insulin, etc), lab draws, wound care, medication administration, etc
-Report any changes to health status to RN that may be observed during visit
-10-12 patients/day, typically about 30min appointments
-Will be seeing the same participants weekly, will be able to grow a relationship with them
60 day reviews will be performed by the RN during an in-home visit, going over care/treatment plan and evaluating for any necessary changes
-Once training is completed (the first week), will not have any requirements to report to the Day Center. LPN's will be able to start/end their days at home (unless they need to go to the Day Center to pick up supplies, training, etc)
-Home visits will all be within a 5 mile radius of the Thornton Day Center, most participants actually reside in the same building so no traveling will be needed - Will be reimbursed 65.5 cents/mile

**OPEN to training folks, finding a team player is most important
- 1 year of experience working with the elderly/frail is a bonus, IV cert is a bonus
-Home care, assisted living, hospice, short term / long term care facilities, etc. **

Schedule:
Flexible Schedules! M-F/Tues - Sat
**Realistically will only do about 5 hours of field work/day, and will be able to log the full 40 hours of work. Aurora, Denver, Lakewood, Loveland areas available!**

Dress code:
Scrubs - Any color/pattern

3-5 Must Have Skills/Qualifications:
Drivers License/auto insurance
Minimum of one year experience working the frail or elderly
Active LPN in CO

Nice to Have Skills:
Home care experience
