LMSW / LCSW

We are looking for LMSWs/LCSWs interested in working within a hopsital setting. If you are an eager individual looking to help their community, and are open to gaining more knowledge within a clinical setting as a case manager this is the role for you!

Overview

• This case manager is not tied down to one speciality will be working with all departments!
• Caseload of 18 patients/beds (med/surg)
• Reviewing admissions
• Care coordination
• Resource guidance: Helping patients understand and access financial and emotional resources
• Will be doing interdisciplinary rounds with physician, RN CM every day at 10 am
• Discharge planning
• Diagnosis explanation: Providing patients and their families with materials that explain their diagnosis
• Life planning: Helping patients plan for life after a hospital stay and determine their next steps
• Will cover the full hospital ( they are not tied to a specific department - they are just covering specific beds in the hospital as a Med/Surg Discharge planner

Monday-Friday 8:00-5:00pm in person

Qualifcations:

• LMSW/LCSW MD license
• Medical experiecne 1+ years within a healthcare field
• Hopsital/clinical/acute care experience preferred
• Discharge planning preferred

Pay: based on experience AND interivew

LMSW- $33-37/hour

LCSW- $37-43/hour
