Job Description

Job Summary:

We are seeking a Licensed Vocational Nurse (LVN) to join a growing healthcare team in North Hollywood, CA. In this role, the LVN will provide in-home patient care services with a focus on underserved populations as part of a Healthcare Plan Partnership Program. LVNs will perform a variety of clinical duties, including point-of-care tests, health screenings, wound care, and more. Orientation will take place at the Torrance, CA location before starting in North Hollywood.

Responsibilities:

  • Perform point-of-care tests (Blood Glucose Testing, nasal swabs, Urineanalysis etc.)
  • Administer EKGs
  • Assist with proctored eye exams
  • Conduct blood draws for lab send-outs (IV/Blood Draw certification required)
  • Provide non-complex wound care and vital signs monitoring
  • Facilitate video visits between patients and healthcare providers
  • Perform health screenings and administer fluoride varnish treatments
  • Ensure accurate documentation of patient visits
  • Work closely with underserved communities, providing compassionate and efficient care in patients' homes

Minimum Skills/Qualifications:

  • Licensed Vocational Nurse (LVN) in the state of California
  • IV certification required
  • Active CA Driver's License with at least 2 years of driving experience and a clean Motor Vehicle Record (MVR)
  • Must be able to lift 40 pounds and handle mobile medical equipment
  • Comfortable with mobile technology and in-home patient care
  • Experience driving long distances if needed

Nice to Have Skills:

  • Bilingual in English and Spanish
  • Familiarity with underserved communities and providing care in home environments
