Job Description

Responsibilities/ Job Duties:

  • Conduct comprehensive behavioral health assessments, diagnoses, and treatment planning for patients.
  • Provide crisis intervention and collaborate with the integrated care team to develop preventive care strategies.
  • Offer brief counseling, psychoeducation, and behavioral health support to patients, families, and staff.
  • Assist the primary care team in developing care management processes, including chronic disease management and patient education.
  • Build and maintain relationships with community resources and behavioral health agencies to ensure continuity of care.
  • Maintain accurate patient documentation and assist with reporting and program evaluation to improve program effectiveness.
  • Manage a caseload of 10-12 patients daily.

Direct Patient Care:

  • Perform comprehensive behavioral health assessments and diagnoses.
  • Develop and implement treatment plans that align with patient goals.
  • Respond to and manage crisis situations with appropriate interventions.
  • Participate in integrated healthcare meetings to ensure collaborative care.
  • Identify "at-risk" patients and assist in creating preventive plans to avoid psychological or physical deterioration.
  • Support primary care teams with care management processes, including disease management, case management, and patient education.
  • Conduct assessments, consultations, and brief interventions for psychological or psychiatric concerns.
  • Educate patients, families, and staff on prevention, treatment techniques, and care options.
  • Monitor the effectiveness of behavioral health programs, identify service-related issues, and recommend improvements.

Program Development and Administration:

  • Build and sustain relationships with community resources and behavioral health agencies to ensure coordinated care.
  • Use electronic community resource directories for patient referrals.
  • Contribute to the development, implementation, and evaluation of special projects and pilot programs.

Minimum Education and Experience Qualification Requirements:

Education and Experience:

  • Master's Degree in Social Work or PhD in Clinical Psychology required.

Licensure and Certifications:

  • Licensed Social Worker (LSW) required.
  • Licensed Clinical Social Worker (LCSW) preferred.
  • Current licensure in the state of New Jersey.

Knowledge, Skills, and Abilities:

  • Proficiency in behavioral medicine and evidence-based treatment approaches.
  • Strong ability to perform quick, accurate clinical assessments in a fast-paced environment.
