Licensed Clinical Social Worker: Palliative Medicine Specialists

The Licensed Clinical Social Worker is a key member of the healthcare team working with patients and
their families to help manage the difficulties of a health or life problem. Utilizes professional social work
skills to assess patients for psycho-social and economic barriers that interfere with the attainment of
healthcare goals. Works closely with Case Managers, Physicians, Nursing staff, spiritual support and
others, as appropriate, to advocate for the patients and families with counseling services, crisis
intervention, stress management, patient and parent coping (as appropriate), bereavement, abuse and
neglect issues and continuity of care. Actively participates in outstanding customer service and accepts
responsibility for maintaining relationships that are equally respectful to all.

Demographic: 18+ (NO pediatrics) // consultative services for those transitioning into palliative care setting, 4-5 patients per day

Schedule: Monday - Friday, 8:30a - 5:00p

Pay: $35-40/hr

Location: Winter Park, FL 33792 - one location, no travel

3-5 Must Have Skills/Qualifications:

• BLS
• MSW
• Licensed Social Worker
• 3+ Years of Experience
• Palliative Care / Hospice / Behavioral Health Therapy
