  1. Responsibilities/ Job Duties:
  • Serve as a dedicated care coordinator for enrolled members, ensuring they receive personalized, whole-person care.
  • Conduct in-home visits (majority of the time) as well as visits to hospitals, shelters, or clinics to assess patient needs.
  • Provide clinical support and health-related education to patients based on their specific conditions and care plans.
  • Collaborate with community resources, healthcare providers, and insurance teams to facilitate access to necessary medical and social services.
  • Utilize telehealth services when necessary to accommodate patient preferences or scheduling constraints.
  • Address social determinants of health, working closely with patients to overcome barriers to care such as transportation, food insecurity, and housing instability.
  • Ensure proper documentation and maintain patient records according to compliance standards.
  • Work alongside Community Health Workers (CHWs), who focus on patient enrollment, while the Lead Care Manager handles direct patient care and coordination.

  1. Minimum Education and Experience Qualification Requirements: Medical Assistant Diploma or CNA
  2. Schedule/ Shift: Monday - Friday 8am - 3pm
  3. Benefits:
