We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team as a Discharge Planner (LCSW)

This position is based in Livingston, NJ, and involves working within a healthcare setting to ensure seamless transitions for patients from the hospital to their homes or other appropriate care settings. The ideal candidate will have excellent interpersonal skills, strong clinical knowledge, and a commitment to patient advocacy.

Requirements: Active LCSW in NJ; 1-2 years of working experience required in the hospital setting or with discharge planning services

Hours: Monday - Friday 8:00 AM - 4:00 PM (no weekends)

Job Type: Travel(contract) or Full Time(perm) options available (depending on preference)

Pay: $30-50/hr

Duties

• Provides direct clinical social work services to patients and their families in a multidisciplinary team approach.
• Completes an initial psycho-social assessment including social work treatment plan.
• Provides individual, family or group counseling, complex discharge planning, bio-ethics case consultation, referrals to community agencies.
• Ensures continuity of care for those patients with a serious illness and maximizes the patient s and family abilities to meet the continuing care needs within their social and financial framework.

Qualifications:

• State licensure required by the NJ State board of social work examiners.
• Licensed clinical social worker (LCSW) depending upon years of social work experience and clinical course work.
• 1-2 of hospital based on discharge planning experience required
