This Socail Worker will oversee 3 location in Little Rock

Daily Responsibilities

Going to the clinic to start your day, meeting with provider to discuss patients
Provider is connecting you with patients when an LCSW is needed
Assessing patients for fall risks, trauma, barriers, family history and physical/mental/behavorial assessments (25% of field visits)
Up to 6 patients a day - visits can be telehealth, in home or in office
Documentation in EMR system
Interactive with Nurse Case Managers, Providers and the rest of the care team
Group Therapy once a week

Patient Population:

Elderly population (Medicare) and/or patients with Depression or Substance Abuse

Pay: $33-41/hr (based on experience)

3-5 Must Have Skills/Qualifications

2+ years of experience as a SW (substance abuse population and/or medicare population preferred)
LCSW/LICSW license in Alabama
Experience in a clinical setting
Comfortable with home visits

Schedule/Shift: M-F: 8a - 5p 25%in the field (you get to make your own schedule with visits!)
