My organization is partnered with a large behavioral health organization who is taking an innovative and dedicated approach to creating services in the most effective and empowering strategy in partnering with clients, families and communities. Position: Interim Case Manager

Schedule: Sunday to Thursday

7am to 3:30pm (closed)

3:00pm to 11:30pm

Tuesday to Saturday

​​​​​​​Description:
Interim Housing programs provide low-barrier, high-tolerance settings for program participants, referred to as guests, who are experiencing homelessness and have a variety of complex health, functional and social needs. These home-like environments are coupled with support and case management services to assist guests with developing a plan to break the cycle of homelessness, stabilize in housing and reintegrate into the community integration.

  • Process and accept referrals from funders.
  • Conduct and complete all new guest intakes in coordination with guests and enter information into HMIS in a timely fashion and within the documentation standards as set forth by the program contract and policies.
  • Conduct a full VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) at admission for all new guests to aid in the Individual Service Plan (ISP) development.
  • Collaborate with other team members and guests to develop Individualized Service Plans (ISP) that address barriers to obtaining services, housing, and appropriate health care needs.
  • Meet with assigned guests a minimum of 1x/week to support progression towards goals identified by guests in their ISP.
  • Provide ongoing case management support with benefits establishment (GR, SSI, TANF, Veterans pension, etc.), obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals and health related assistance as identified on the ISP.
  • Provide referrals and linkages to primary and behavioral health services and other supportive services.
  • Collaborate with referring organizations to coordinate guests' care and ensure guests receive the services and support they need to achieve and maintain health, mental health, and housing stability.
  • Assist with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing.
  • Assist and educate guests as needed about care and safety regarding their animals.
  • Advocate with prospective landlords and identify appropriate permanent housing options for guests.
  • Provide ongoing information, referrals, linkages, and advocacy for all other identified needs.
  • Provide linkage, referrals, and case management support to interim guests.
  • Facilitate psychosocial rehabilitation groups for guests as required.
  • Use and develop community resources to broker and link guests to services.
  • Provide all guests with a home-like environment for the duration of their stay in Interim Housing.
  • Collaborate with the case management and operations teams to ensure that the facility is clean and safe.
  • Ensure that guests meals are served at the established times and support kitchen staff as required.
  • Assist all volunteers by providing all necessary supports to ensure that they have a valuable experience.
  • Promote and follow interim housing established values, guidelines, and rules.
  • Participate in staff meetings and assigned trainings.
  • Maintain documentation standards as set forth by the program contract and policies, including maintaining compliance with HIPAA policies and practices.
  • Complete progress notes on every face to face or telephone contact with guest or collateral contact by the next business day at 5 pm.
  • Must demonstrate a commitment to diversity, equity, and inclusion through inclusive service delivery, continued learning and development, modeling inclusive behaviors, contributing to an inclusive work environment, proactively mitigating bias in internal and external systems as well as interactions with internal and external stakeholders.
  • Perform all other duties as assigned or required.

WHAT YOU BRING

  • Ability to maintain and execute confidential information according to HIPAA standards.
  • Demonstrated knowledge and/or experience with case management techniques, including critical time intervention, harm reduction strategies, crisis intervention techniques, and motivational interviewing.
  • A high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
  • Ability to work independently and within a team environment and exercise mature judgment.
  • Ability to provide community and in-home based services on a regular basis.
  • Strong written and verbal communication skills.
  • High level of motivation and self-starting demeanor with the ability to coordinate multiple projects and tasks simultaneously in a high-pressure environment health and safety
  • Observe and abide by companies health and safety policies and procedures.
  • Perform all job duties in a safe manner.

Preferred qualifications

Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.

Minimum qualifications

All levels of education and experience are welcome.

Successful completion of background screening & drug test.

Ability to work flexible hours which may include evenings or weekends.

Ability to obtain CPR/First Aid certification through PATH.

Driving is an essential function of this position. To meet the requirements of for this position, the incumbent:

Must hold a valid CA Driver's License.

Must possess their own vehicle and provide proof of ownership.

Must provide proof of insurance coverage.
