Job description

Medix is hiring for Intake Registered Nurses!

Roles open in the Bronx and Brooklyn.

Full-Time

Hybrid schedule (onsite once a week or once every two weeks after training)

Schedule: Mon-Fri 9am-5pm

Pay: $95k-105k/yr

Description:

Ideal candidate will be responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their family.

Responsible for knowing require outreach activities within the community and the assessment of eligibility and safety of potential members.

Collaborate with other members of the Intake/Outreach team and the clinical team to assure the appropriate provision of services to new members.

Communicate with potential members, family, referral sources and physicians to assure a seamless transition to managed long term care.

Qualifications:

Must be a self-assured professional with a current New York State Registered Nurse License required. Bachelor's degree in Nursing required.

One (1) year experience in an acute care setting required. Must have UAS- NY experience.

Home care and management experience preferred. MS Office required.
